  • 2.4. The business license (if this business is licensed on a regular basis).
  • 2.5. Contact person of a Copyright Owner (name, job title, phone number, email).
• 3. The information of the complaint sender:
  • 3.1. Name.
  • 3.2. Job position.
  • 3.3. Phone Number.
  • 3.4. E-mail.
  • 3.5. Copy of the designation of the authority from the Copyright Owner ( not needed in case the person sending the complaint is the Head of the Copyright Owner Company).
  If the complaint is sent not by the Copyright Owner but by its authorized representative – Individual Person, he should submit the copy of the designation of the authority from the Company to the authorized individual (not needed in case the person sending the complaint is the Head of the Copyright Owner Company).
• 4. Claim Letter Information:

  • 4.2. The complete description of the copyright abuse situation (why the spread of this information is prohibited by the Copyright Owner).
